The 10 Stage Transition Pathway

Orbis Support adhere to a clear process, showing how we can set up and provide a service from start to finish. This process ensures every detail is covered and which allows us to provide exceptional support from the first day of a service commencing.

Stage 1

Referral Received

Stage 2

Initial Assessment

Orbis will assess the person(s) and any other relevant people

Stage 3

Written Assessment

Orbis will complete a written assessment confirming service viability, proposed service design, transition plan and costing’s (if appropriate) and return to referrer

Stage 4

Referral Decision

Orbis will wait for decision from referrer. If the decision is ‘No’, the Transition Pathway ends. If ‘Yes’ a contract, or written confirmation will be required from authorised person which agrees to the proposal and costing’s

Stage 5

Transition Plan

A transition plan with confirmation of the transition lead from Orbis, dates, times and objectives will be set and shared with all relevant stakeholders. At this stage, Orbis will decide who will be managing the person (this person is often the transition lead, but doesn’t have to be)

Stage 6

First Date of Transition

First date of transition with the person supported begins. Concentration on the development of the relationship is key at this point

Stage 6.1

Documentation

(To be completed alongside the following stages, but must be completed and agreed to prior to the service beginning) – Relevant documentation, including risk management plans, are created (if applicable). The sharing of the documentation with relevant stakeholders will be practiced throughout the transition and beyond. In particular, risk management plans will be signed off and agreed by relevant stakeholders before implementation

Stage 6.2

New Support Team

An advert  is placed on Recruitment websites/job centres etc. for the new support team. Advert will be specific to the needs of the post, for instance the person’s likes and dislikes. This increases the likelihood for compatibility with the person supported and increases the chances of a positive relationship forming

Stage 6.2

Property Search

Begin search for property based on the service design created in the assessment. Ensure all relevant stakeholders are involved in this process

Stage 7

Interviews

After exploring applications, invite potential employees in for first stage interview. Within this interview, the applicant is told they will have to have a second interview with person supported if they are successful

Stage 7.1

Second Interviews

Person supported completes second stage interview. They will then ultimately decide if they take the person(s) on. It is important to note that the person supported doesn’t have to interview if they don’t want to, although they must be given the chance. If they don’t want to, they can nominate a friend or family member to do it, or they could just ask if Orbis can complete the whole process (in which case only one interview is required)

Stage 7.2

Setup

Once the new employee has signed their contract, Orbis will apply for their DBS’s and references. All new Orbis employees to be set up on the bespoke Orbis database and assigned a personal email address

Stage 7.3

First Meeting

The support team to meet with the person supported, this can only happen after DBS received

Stage 7.4

Training

Orbis training will commence to the new support team with an induction, specialist training to the role and online learning

Stage 8

Visit Properties

Visit potential properties (this can be done with the person supported if appropriate) If the person supported can’t attend, but wants to be involved in the decision making process, take photos of the properties, and show them to the person supported. They can then choose the property they’d like to live in. If the person supported (or a representative) doesn’t want to do this, Orbis will make the decision (along with other stakeholders if applicable)

Stage 9

End of Old Support

The person supported is supported to say goodbye to his current providers/care givers

Stage 10

The Service Begins!
